Problem with new threads

For the CHOICE classes we use file uploads, but there is a case where the upload process fails.  When you very first create a thread, you are offered the opportunity to upload a file, but if you attempt an upload nothing happens.  Furthermore, after you attempt the upload, when you "save" that first posting for the new thread the system creates two threads instead of one.
