Observer Database

I am new to this process, although much of the logic is pretty clear to me. In the course of setting up I find that my ability to consider and record my local work data is pretty weak. Does anyone have a database (maybe in Access, Excel or Filemaker) that you could share? I can imagine the issues but need to set up the process correctly.

Any advice will be most gratefully accepted.
